Carilion Clinic in partnership with Healthy Roanoke Valley and a local team of cross-sector partners is currently conducting triennial 2018 Roanoke Valley Community Health Assessment (RVCHA). We are interested in learning about the health of all members of the Roanoke City, Salem City, Botetourt County, Craig County and Roanoke County communities, specifically those underserved/vulnerable populations disproportionately impacted by the social determinants of health including: poverty, race/ethnicity, education, and/or lack of insurance. Here’s a link to the 2012 and 2015 Community Health Assessments: https://www.carilionclinic.org/community-health-assessments if you want to look at the work that’s been done before.
Please help distribute and collect surveys in the community. Our goal is 4,000 surveys filled out by February 28, 2018!
Individuals and families you serve may be a good fit for this. You can actively distribute the surveys, or have them available at your place of business while people wait. The survey takes about 15 minutes to complete. Survey participants also have a chance to enter a raffle for a $50 Kroger gift card (must be 18 or older to take survey).
Surveys can be completed:
- By paper: contact Amy Michals at firstname.lastname@example.org for print surveys, raffle tickets, Spanish surveys or fliers and posters. The materials are ready, all we need is a distribution and collection mechanism.
- Online: https://www.surveymonkey.com/r/2018CHA
- By phone: 888-964-6620
