VWPD General Complaint Form

Use this form to report officer misconduct or any other substandard performance.

**IMPORTANT**
In order to ensure complaints can be fully investigated we must have valid contact information.

Describe the nature of your complaint. Please be as specific as possible with dates, times, locations, etc. If the name of the officer is unknown provide as much descriptive detail (height, hair color, etc) as you can remember. If you're including all or part of a conversation try to be as close to verbatim as possible.
